Patient care is where the rubber hits the road, and doctors are often blamed when a patient doesn’t get exactly what they want. The previous entitlement system allowed the patient to get an MRI or other expensive test on demand, but in this new system a test or medicine may not add value enough to justify the cost. The cost of care I provide is constantly being measured by metrics in electronic medical records and I’m tiered against my peers. It’s frustrating, because sometimes I feel as though I don’t have a patient’s best interests at heart and discussions on value and cost breaks down patient trust.
